Middle cerebral artery effects upper or lower limbs more?
Upper limbs, Supplies more laterally to the central sulcus. Thinking about the cerebral homonculus, the lower limbs are in the central sulcus and then the rest of the body is more lateral
How does aspirin work?
Inhibits COX1/2 from producing thromboxane A2 from arachidonic acid. TA2 is used for platelet activation. Therefore platelet activation is inhibited
How does clopidogrel work?
P2Y12 receptorinhibitor. Stops ADP binding to receptor, which would have activated platelet.
How do Dipyridamole work in anti-platelet therapy?
Inhibition of Phosphodiesterase therefore stopping degradates cAMP which inhibits platelet activation.
How do GPIIb/IIIa receptor antagonist work in anti-platelet therapy?
Blocks vWF and fibrin binding to receptor which would have otherwise lead to aggregation of two adjacent platelets.
What blood test should be done when suspecting anaphylaxis?
Mast cell tryptase. Two measurement should be taken as it rises and falls quickly after anaphylaxis
What does a ABCD2 score mean?
>4 = referral within 24 hours <4 = referral within 1 week
What is dysphasia and what types are there?
Dysphasia is a language disorder
Types are: receptive or expressive
What patterns of muscle weakness are there?
Pyramidal
Myopathy
Distal
Fatigue
What is the pyramidal distribution of muscle weakness?
Lower limb flexors and Upper limb extensors are weaker
What would point towards a organic cause for 1st episode psychosis?
Disorientation
Fever
Urine incontinence
