What are the indications for a PPM insertion?
A
- persistent symptomatic bradycardia
-Mobitz type II AV block - sinus pause >3sec
- trifascicular block
3rd degree AV block + RBBB + LAFB or;
3rd degree AV block + RBBB + LPFB

Why does papillary muscle rupture after MI require valve surgery definitely?
A
Papillary muscle rupture –> mitral valve regurgitation –> acute left ventricular failure –> initial treatment with sodium nitroprusside to decrease afterload –> definitely valve surgery

What is pulsus paradoxus?
A
Seen in cardiac tamponade. Decrease in palpable pulse and hypotension on inspiration.

What is Kussmaul’s sign?
A
Seen in constrictive pericarditis. Inspiratory increase in JVP.
