what is an aneurysm ?
weak vessel wall - abnormal dilatation
what are the 2 types of aneurysms associated with the brain ?
saccular or berry aneurysms
charcot bouchard aneurysms
what are the charcot bouchard aneurysms ?
aneurysms that happen in the lenticulostriate vessels
associated with chronic hypertension diabetes and smoking

APKD
ehlers danlos
aortic coartication
old age

what is the result of rupture of a berry aneurysm ?
subarachnoid hemorrhage
bleeding into the CSF
where does the bleeding occur in subarachnoid hemorrhage ?
between the arachnoid and the pia mater
what is the presentation associated with subarachnoid hge ?
described as the worst headache of their life
sudden onset of symptoms
fever
stiff neck
what is seen on spinal tap in association with subarachnoid hge ?
xanthocromia
what artery is ruptured in subarachnoid hge ?
Acom or Pcom
what is the classic sign seen in subarachnoid hemorrhages due to pcom ?
ipsilateral blown pupil
CN 3 palsy
what is the most common risk factor associated with SAH ?
hypertension
what is the appearance on CT of a SAH ?
starfish appearance in the middle of the brain
or described as blood in the cistern
what is the treatment for SAH ?
FFP for patients on warfarin
nimodipine can prevent vasospasm
if the pupil is spared THIS IS NOT AN ANEURYSM
then it is probably associated with DM and is usually painful
