What is epididymitis?
Inflammation of the epididymis - a coiled tube that stores and carries sperm in the back of the testicle , often caused by infection.
What are the common symptoms of epididymitis?
Scrotal pain, swelling, and tenderness.
When is emergency hospital admission required?
For complications of hernia or epididymo-orchitis (e.g., sepsis, abscess), or haematocele from trauma.
When should you urgently refer to a sexual health clinic?
If epididymo-orchitis is suspected to be due to an STI.
What self-care is recommended for epididymitis?
Rest, scrotal support, and analgesia (paracetamol/NSAIDs).
What is the treatment for STI-related epididymo-orchitis?
Ceftriaxone 1 g IM single dose + doxycycline 100 mg twice daily for 10–14 days.
What if doxycycline or cephalosporins are contraindicated?
Use ofloxacin 200 mg twice daily for 14 days.
What is the treatment if an enteric organism is suspected?
Ofloxacin 200 mg twice daily for 14 days or levofloxacin 500 mg once daily for 10 days.
What is the treatment if both STI and enteric organisms are suspected?
Ceftriaxone 1 g IM + ofloxacin 200 mg twice daily for 14 days.
