Flashcards in Heme Drugs Deck (21):
1

Warfarin MOA

inhibits epoxide reductase (reduces Vit K to activate Factors II, VII, IX, X & Proteins S & C) (inhibits pro-coagulation factors)

2

Heparin MOA

activates antithrombin (which inhibits activated forms of procoagulation factors: Factor IIa (thrombin), VIIa, IXa, Xa, XIa, XIIa) (activates anti-coagulation pathway)

3

tPA

thrombolytic catalyses reaction of plasminogen to plasmin which cleaves the fibrin mesh into fibrin degradation pathways.

4

Aspirin

Inhibits cycloxygenase (enzyme that transforms arachadonic acid to thromboxane A2) decreases aggregation of platelets

5

Clopidogrel & Ticlopidine

inhibit ADP-receptor-induced expression of GpIIb/IIIa (decrease GpIIb/IIIa expression on platelet surface) decreases aggregation of platelets/ linking

6

Abciximab

inhibits GpIIb/IIIa directly cannot bind fibrinogen and aggregate platelets

7

Hereditary Spherocytosis

Splenectomy is curative (post-splenectomy will have howell-jolly bodies)

8

Paroxysmal Nocturnal Hemoglobinuria

Eculizumab

9

Sickle Cell Disease

Hydroxyurea (increases HbF) bone marrow transplant

10

Lead poisoning

Dimercaprol EDTA

11

Acute intermittent porphyria

glucose + heme (inhibits ALA synthase)

12

Sideroblastic anemia

pyroxidine (B3) (co-factor for D-ALA synthase-- which is deficient in hereditary (XLR) sideroblastic anemia

13

von Willebrand's disease

DDAVP (desmopressin) releases vWF protein from endothelium

14

Packed RBCs trasnfusion effect

inc Hb & O2 carrying capacity

15

Packed RBC transfusion indication

severe anemia acute blood loss

16

Platelet transfusion effect

inc platelet count by ~5,000/mm3/unit

17

Platelet transfusion indication

significant bleeding (stop) thrombocytopenia qualitative platelet defect

18

Fresh frozen plasma transfusion effect

increases coagulation factor levels

19

Fresh frozen plasma transfusion indication

DIC warfarin OD cirrhosis

20

Cryoprecipitate transfusion effect

increases fibrinogen, fibronectin & factors VIII, XIII & vWF

21

Cryoprecipitate transfusion indication

coagulation factor deficiencies-- esp involving fibrinogen & factor VIII (hemophilia A)
