KOH

Dissolves cellular elements, preserves fungi & protozoa. Different species present under microscope in varied ways

Gram Stain

Identification of + or - bacteria to determine initial antibiotic therapy. ID presence of bacteria in typically sterile media

Diascopy

Identification of blanching versus non-blanching skin issues. Using microscope slide.

Bacterial (wound) culture

Swab of wound to guide abx therapy to targeted organism and susceptibility to abx. Must swab prior to initiation of therapy

Herpes serology

Swab from active lesion, unroof lesion and swap at deepest point. Allows different ion between types of viruses

Viral culture

Swab site of active lesion to determine which type of virus is causing the infection

Fungal culture

Confirm or establish fungal or yeast infection. Takes 2-8 weeks. Good for infections that aren't resolving with standard therapy

Acetowhitening

Detection of warts using acetic acid (vinegar). Warts appear white.

Tzank smear

Gold statndard in herpes dx. Deroof blister, fix and stain. Appear as multinucleate or giant cells under microscopy. Cant differentiate between subtypes

Cellulitis Imaging

Ultrasound- accumulation of fluid give cobblestone appearance. CT if extensive infection

Necrotizing fasciitis

T2 weighted MRI- only indicated to rule out. See extent of damage to facial plane. If high suspicion schedule with surg.

Gas gangrene

CT scan. extraluminal and deep gas shows up black

Osteomyelitis

MRI- not specific- evaluate extent of soft tissue involvement and debridement

Infections Arthopathies

MRI- prompt detection of spinal problems and bony involvement

Arthocentesis- indications

unexplained swelling, potential infections or inflammatory process- diagnostic or therapeutic

Arhtocentesis- Contraindications

cellulitis or broken skin, anticoagulant therapy, suspected bacteremia, unstable joint, inaccessible
