Inflammation of fascia caused by weight bearing and activity

Necrotizing fasciitis- Eti

Strept most common, often polymicrobial. Two types.
1- polymicrobial, following surge, bowel perf in extremities & abdomen
2- monomicrobial, GAS or MRSA, typically healthy patient or DM/ alcoholic

Necrotizing fasciitis- Sx

Risk factors + severe spontaneous pain, bullae, ecchymosis, gas by exam and cutaneous anesthesia, dusky blue color, constitutional symptoms

Necrotizing fasciitis- Dx

Lab risk indicator- C reactive protein, WBC count, hemoglobin, Na, creatanine & glucose.

Necrotizing fasciitis- Tx

Immediate surgical debridement, broad spectrum abx

Myositis- Eti

S. aureus and S. pyogenes. Muscle infection following trauma to tissue, microbial infection or strain

Myositis- Sx

Muscle fatigue & weakness- slow and progressive- coordination issues

Myositis -Dx

Muscle biopsy

Myositis- Tx

Corticosteroids, immunosuppresants PT, exercise, heat

Gas gangrene- Eti

Injury involving interruption of blood supply- clostridium

Gas gangrene- Sx

Foul smelling, blisters & gas bubbles, rapid tissue destruction

Gas gangrene- Dx

Imaging & gram stain

Gas gangrene- Tx

Amputation and removal of infected tissue, penicillin
