Apex (apices)
bilaterally (2cm superior to medial 1/3 of clavicle)
Superior Lobes anterior
(2nd intercostal space mid clavicular line) and posterior (Between C7 & T3)
Inferior Lobes bilaterally anterior
(6th intercostal space, mid-axillary line) and posteriorly (between T3 & T10)
Middle lobe right anterior only
(4th intercostal space mid-clavicular line)
R and L upper lobe
Anteriorly: place stethoscope above clavicle, and on ribs 2-4
Posteriorly: above spine of scapular, and at level of T3
L upper lobe
anterior surface of ribs 4-6
R middle lobe
anterior surface of ribs 4-6
R and L lower lobe
Laterally: ribs 6-8 in mid-axillary line. Anteriorly - ribs 6-8, 45 deg down from nipple. Posteriorly - T8-T10. Apical lower lobe: T3-T7 posterior surface.
Auscultation
Compare one side to the other looking for asymmetry and note the location and quality of the sounds you hear
