What is MDH?
MDH is a standard form that states the health of the vessel and its contents.
What does the MDH declaration request?
The MDH declaration requests several pieces of information regarding the vessel’s health and travel history.
What is required regarding sanitation?
A valid Sanitation Certificate or exemption is required.
What information about affected areas is requested?
It asks whether the vessel has visited any affected area as declared by the World Health Organisation.
What travel history is requested in the MDH?
A list of ports visited within the last 30 days or since the commencement of the voyage, whichever is shorter.
What crew and passenger information is needed?
A list of crew members and passengers that have joined the vessel since the commencement of the voyage or within the last 30 days, whichever is shorter.
What information is requested about deceased persons?
Any persons that have died during the voyage as a result of illness or other than accident.
What disease information is required?
Any cases of disease during the voyage that are/were infectious in nature.
What details about illness on-board are requested?
Any illness on-board, its nature, and if any medical practitioner was consulted along with the treatment provided.
What sanitary measures may be reported?
Any sanitary measures such as quarantine that were applied.
What information about animals is requested?
Any sick animals on-board.
What information regarding stowaways is needed?
Any stowaways present on the vessel.
