Morphine MOA

A
  • Elevates pain threshold by acting on the pain receptors in the CNS.
  • Depresses respiratory and fear/anxiety centers in the brain.
  • Increased vasodilation reduces preload and afterload, decreasing myocardial oxygen demand.

Morphine Adult dose

A
  • Pain management:
  • 2 – 4 mg slow IV/IO push; may repeat with 2 – 8 mg in 5 – 15 min. as needed
  • 0.1 mg/kg IM (max dose of 5 mg)
  • Acute chest pain with ST-elevation:
  • 1 – 5 mg slow IV/IO push; may repeat as needed

Morphine Contraindications

A
  • Known hypersensitivity
  • Hypoventilation or hypoxia despite high-flow oxygen
  • Head injuries, seizures, or brain tumors
  • Acute or severe asthma (relative)
  • Delirium tremens
  • Cor pulmonale

Morphine Adverse reactions

A
  • CV: bradycardia, tachydysrhythmias, orthostatic hypotension
  • Resp: respiratory depression or apnea
  • CNS: confusion, headache, seizures, syncope
  • GI: nausea and vomiting
