189. Ensure the following stakeholders sign the Drill Site Approval Form:

• Drill site approval applicant
• District Operations or Functional Commander (or another delegated Commander)
• Employee HSR
• Specialist instructor/operator (where required).

When conducting debriefs:

• Ensure all debriefs are completed by the OIC of the appliance or Incident Controller.
• Document the debrief using the FRV Debrief Template.
• Conduct the debrief after the event/incident as soon as practicable.
• Ensure the following areas are covered in the debrief.
  o Acknowledge and highlight successful outcomes and processes.
  o Provide the opportunity for personnel to discuss any issues or concerns and capture relevant information in regard to operations.
  o Address any health and safety issues.
  o Initiate action/s to resolve any significant issues.
  o Identify opportunities for improvement which may result in lessons to be learned for the organisation.
• Ensure the following occurs when conducting a debrief:
  o Avoid focusing on individual performance or issues that can be resolved at a local level.
  o Do not confuse opinions with observations.
  o Focus on the performance of systems and processes as a whole.
  o Capture key points and observations which may include successful outcomes and/or opportunities for improvement.
• Refer to the Debrief Checklist (Appendix 1) to assist in the coordination or facilitation of a debrief involving multiple crews and FRV personnel.
