4. Provide the following information: Hospital destination, source patient’s name, date of birth, and medical record number, detailed incident description, contact phone number (employee must answer follow-up calls).
5. Complete and submit: Infectious Disease Exposure Report (FRD-314), Injury Packet, including:
Supervisor’s Report of Work-Related Injury (FRD-071).
Battalion Chief’s Investigation Report (FRD-315A).
Injury/Accident Narrative Supplement (FRD-317) .

Immediate Supervisor Actions:
1. Separate the Employees
• Attempt to defuse the situation immediately if safe.
• Separate individuals involved and direct them to different areas of the workplace.
• Supervisors are not required to physically intervene if it puts them at risk.
2. Call Police If Necessary
• Report the incident up the chain of command immediately after police are notified.
3. Report the Incident
• All incidents involving threats or violence must be reported through the chain of command for further investigation and possible discipline.
• Complete a Workplace Violence Incident Report Form (FIN-03).
4. Investigate & Document
• Supervisors are responsible for conducting an initial investigation.
• Collect detailed statements from both parties and any witnesses.
• Document all details thoroughly for the Professional Standards Office (PSO) to review.
5. Coordinate with PSO
• The PSO may assist in interviewing witnesses and involved parties to get a complete and objective account.
• Submit all required documentation.

Pregnancy Notification Process (Supervisor Steps)
1. Receive Notification Respectfully
• The employee must submit written confirmation of pregnancy from their Primary Care Physician (PCP).
2. Encourage Medical Consultation
• Advise the employee to schedule an appointment with the Public Safety Occupational Health Center (PSOHC).
• The employee must provide their PCP with Appendices 1, 2, and 3 from the Pregnancy Manual for review and signature.
3. Discuss Operational Status
• If the employee opts to leave the field or is unable to perform essential duties:
• The employee notifies their supervisor of this decision.
• The employee can request light duty assignment.
• The employee must submit an FRD-074 (Medical Status Report) and schedule PSOHC evaluation for light duty approval .
4. Coordinate Light Duty & Leave
• If light duty is approved, the supervisor helps coordinate:
• FMLA paperwork (if applicable).
• Assignment to light duty per SOP 02.03.06.
• Completion of required forms (FRD-158 Work Status Notification).
5. Maintain Confidentiality
• Keep all medical information confidential per FRD policy.
• Supervisors must NOT disclose the employee’s pregnancy status beyond those with a “need to know.”

Officer’s Required Actions:
1. Immediately Notify the On-Duty Safety Officer (SAFO)
• SAFO determines whether it is a Risk Exposure or Suspected Non-Risk Exposure.
2. Document the Incident Thoroughly
• Complete the Good Samaritan Citizen Exposure Information Intake Form (FRD-399-A).
• Submit it to the SAFO.
• Provide the citizen with a copy of the completed form.
• Inform the citizen their exposure will be reported to the Fairfax County Health Department, as required by Virginia law (§ 32.1-45.1, Deemed Consent) .
3. Coordinate with Health Department & Infection Control Officer (ICPO)
• The ICPO and SAFO will coordinate with the Fairfax County Health Department for any needed follow-up, including source patient testing or post-exposure prophylaxis.
