Why does digoxin need a loading dose and why does the maintenance dose need reducing in renal failure?

Long Vd, so long half life, so will otherwise take a week to reach a steady state
Predominantly excreted by the kidneys

What is the principle of treatment for a paracetamol overdose?
A
Increasing toxic levels of NAPQI due to saturation of normal metabolism. NAPQI depletes glutathione levels. Replace glutathione with acetylcysteine (parvolex)
