What is vestibular neuronitis (VN)? And what are the common symptoms?
Is a self-limited inflammatory disease effecting the vestibular part of cranial nerve 8. Kan takes hours to days.
Nausea, vomiting, gait problem difficulty with balance, vertigo. Usually no headache.
NO HEARING LOSS
A common viral cause of VN?
a dormant HSV
Most common type of peripheral vertigo?
- BPPV
- Meniere’s
- VN
Who is more susceptible to VN?
There is no gender difference but mostly middle age adult >35-40
Clinically what’s the main difference btw VN, BPPV and manières?
VN is is not EPISODIC unlike BPPV or manières. Symptoms gradually gets worsen reaching the peak in 24-48 hours and then over several days subside
Clinical differentiation btw VN and labyrinthitis?
Labyrinthitis has the same symptoms as VN but additionally there is UNLIATERAL HEARING LOSS
Clinical difference btw Labyrinthitis and Meiners?
Meiners is EPISODIC btw 20 min to several hours (<12 hours) which labyrinths is not episodic
How to differentiate peripheral from central causes of vertigo?
Usually episodic symptoms means mostly peripheral while constant worsening means central. Sadly VN has constant worsening symptoms. DO HINTS test to dedifferentiate peripheral from central vertigo.
