1) Residual hyperlipidemia - lipoprotein A 2) infection: bacteria (chlamydia pneumoniae, h pylori, periodontist) and virus (HSV, CMV, EBC, HCV, enteroviruses) 3) diet - trimethylamine oxide (TMAO) 4) environmental: organic pollutant (dioxins, polychlorinated biphenyls), air pollotuion, heavy metals (arsenic, cadmium, mercury)
