imbalance in fibrotic repair system of lungs = lay down unrequired scar tissue
chronic inflammatory infiltrate (neutrophils) causes fibrosis in alveolar walls and macrophages

symptoms

progressive exertional dyspnoea
dry cough

signs

clubbing
bibasal crackles on auscultation
dry cough

diagnosis

CXR - bilateral infiltrates, fine inspiratory crackles
HRCT - bilateral lower zone fibrosis/shadowing - ground glass
restrictive PFT and low TLCO

treatment

oral steroids
immunosuppressants (azathioprine)
O2 if hypoxic
pulmonary rehab
lung transplant in young patients

10

complications

respiratory failure
most patients dead in 5 years
