Why is time critical in STEMI?
Because time to coronary reperfusion affects the amount of myocardial damage and risk of death.
What is the primary goal once STEMI is identified?
To expedite coronary reperfusion, whether via PCI or PHT.
What is the intended destination for all STEMI patients?
A PCI centre.
When does timing to PCI facility start?
From the time ECG changes consistent with STEMI are identified by a PHT-endorsed and equipped paramedic.
What should an ALS paramedic do if the ECG shows STEMI, but they believe the monitor is incorrect?
Contact the AV Clinician before administering thrombolysis.
Can MICA paramedics override monitor interpretation?
Yes – using clinical judgement and ECG interpretation skills, but can consult AV Clinician if unsure.
Where should patients go after prehospital thrombolysis?
To the closest PCI facility, in consultation with the Clinician.
What if distance or resources prevent direct transfer to a PCI centre?
Consider: 1) AAV for transfer, or 2) Interim transfer to a registered ED facility for ARV coordination.
What are the requirements for an interim destination?
It must be a registered ED capable of temporary thrombolysis care while awaiting ARV to PCI.
