11. Acute Diarrhea-Non-inflammatory 1 Flashcards

1
Q

Mark a 27 year old man goes to Mexico for his Bachelor party. During his trip he ate and drank lots of local foods. He is now suffering from non-bloody diarrhea when he returns home and goes to the doctor. Lab test are done showing and infection by a gram negative, catalase positive motile bacteria. What is true of the most likely infectious agent?

A. Activates Guanylate cyclase
B. Effaces microvilli
C. Causes destruction and necrosis
D. Has AAF
E. Grows on TCBS

RFA

A

Ans. A. Activates Guanylate cyclase- this is by ST one of the virulence factors of ETEC also know as travelers diarrhea the other is LT which activates adenylate cyclase and is similar to Cholera toxin

B. Effaces microvilli- the is characteristic of EPEC or “infantile diarrhoea”

C. Causes destruction and necrosis- this would cause bloody diarrhea this is a characteristic of EIEC

D. AAF- aggregative adherence fimbriae is a characteristic of EAEC which would cause bloody diarrhea

E. Grows on TCBS- would be cholera, rice water like stool is the buzz word for cholera

Q

A 22 year old college student is low on cash and out of desperation reheat up some 4 day old chinese food leftovers. His budging feast includes: sweet and sour chicken, broccoli beef stir fry , white rice, egg drop soup and an egg roll. Two hours eating the meal he is throwing up. What is the toxin responsible for this reaction?

A.ST Neurotoxin
B. Cholera toxin
C. LT Toxin
D. Botulinum toxin
E. AC Toxic

RFA

A

As. A. ST Neurotoxin- The patient is infected with Bacillus cereus specifically the Emetic form:vomiting, time within 4 hours of ingestion. And the toxin responsible in the ST neurotoxin

the other type is diarrhoeal: watery diarrhoea, 8-16 hours and is caused by LT toxin

Q

A 22 year old female is on ampicillin, gentimycin, and clindamycin to treat acute polynephritis. 24hrs after the start of her antibiotics, she develops watery diarrhea and severe abdominal pain. She is afebrile. Laboratory testing shows the prescence of numerous gram positive non-motile rods. The causative organism is also…. (MG)

A. Non-spore forming
B. Facultative anaerobe
C. PAS positive
D. Anaerobe
E. Flagellate
A

D. Anaerobe

Q

Two landscapers are working hard to create new flower beds in a client’s backyard. They stop to eat lunch and sit down on the soft soil they just brought in. Some of the soil gets on their sandwiches but they don’t care and continue eating. 16 hours later, they are experiencing diarrhea and abdominal pain. Lab evaluation shows the presence of gram positive rods arranged in chains. What is another feature of the causative organism? (MG)

A. Facultative anaerobe
B. Aerobic
C. PAS positive
D. Non-spore forming
E. Producing hydrogen peroxide
A

B. Aerobic

Dx. is diarrhoeal Bacillus cereus

We usually only have stems that focus on contaminated food since it’s a type of food poisoning, but the bacillus cereus habitat includes soil so eating soil could cause it to be ingested.
