Primary HA

have no organic cause and include migraines, tensionlike and cluster

Secondary HA

have an organic cause and need medical management

Migraine HA sx

unilateral pain behind one ear or eye, photo/phonophobia, N/V, last 4-72 hours

Migraine HA categories

with or without aura or atypical

Prodromal stage (with aura)

awareness of findings hours to days before onset: irritability, depression, food cravings, diarrhea/constipation, and frequent urination.

Aura stage (with aura)

develops in minutes or hours (can be tingling or visual disturbances)

Second stage (with aura)

severa throbbing HA w/ N/V, vertigo and drowsiness

Third stage (with aura)

4-72 hr, dull HA, aura may continue while pain subsides

Recovery stage with pain and aura subsiding

muscle aches, activity worsens pain, stiff neck and head

Migraine HA without aura

unilateral pulsating pain, 4-72 hr, often in morning

Atypical migraine HA

longer than 72 hr, neurological manifestations for 7 days, neuroimaging may find infarct, does not fit other criteria

Triptan preparations ex

zolmitriptan [Zomig], sumatriptan [Imitrex], eletriptan [Relpax]

vasoconstriction

Ergotamine preparations with caffeine ex

Cafergot, dihydroergotamine [Migranal]

Ergotamine preparations with caffeine function

narrow blood vessels and reduce inflammation

Three R approach teaching

Recognize migraine manifestations.
Respond and seek provider.
Relieve pain and manifestations.

Cluster HA sx

Intense, unilateral, nonthrobbing, last 30-120 min, can radiate to forehead, temple, or cheek
Occur daily around the same time for 4-12 weeks then remission 12 mths
No warning signs

men between 20-50

Cluster HA tx

same as migraine but O2 @7-10L/min may help at HA onset
