resilience can be caused by
social and economic factors.

• Causes of poor health, well-being and
resilience, e.g. loneliness, lifestyle
behaviours and finances, are addressed to
promote long-term positive outcomes.
• Empowerment of individuals.
Promotion of a holistic approach.
Social prescribing and care navigation Delivery:
Cwmtawe Social Prescribing Service (part of Abertawe Bro Morgannwg University Health Board) is one example. Many health boards in Wales have similar projects.
