Definition, localization, onset and complications of Intussusception

A

○Telescoping of intestines, proximal segment into a distal segment.

○Especially occurs at ileocecal junction, and children usually have it from Meckel diverticulum.

○ Usually occurs between age 3 months and 3 years.

○ Leads to ischemic/gangrenous bowel, perforation.

Symptoms of Intussusception

A

Intermittent abdominal pain due to cut off blood supply
Patient may draw legs to chest to ease the pain
Triad : Colic abdominal pain, vomiting, bloody stool.
Currant jelly stools are from the blood and mucus.

Risk factors of Intussusception

A

Meckel diverticulum.
Henoch-Schonlein purpura.
Celiac disease.
intestinal tumors.
Rotavirus vaccine
Recent viral infections (adenovirus).

Diagnosis of Intussusception

A

Target sign on ultrasound :
Outer hypoechoic (dark) ring, representing the intussuscepted bowel, and an inner hyperechoic (bright) ring, representing the mesenteric fat and vessels that have been invaginated.

Treatment of Intussusception

A

■ Air or saline enema: air enema is often safe and successful to reduce.

■ Surgical removal of lead point.
