The Rx CQI (continuous quality improvement) peer review file contains 2 reviews.
GOOD: CQI file exists with either a quarterly evaluation or incident for the 2 most recent completed quarters. Not handwritten, but electronic. Rxs requiring State Board review not audited. OPPORTUNITY: CQI file not located or doesn’t contain either evaluation or not signed.
Rx adheres to the partners and posting of professional licences SOP.

The warehouse purchase vs. dispense report is reviewed monthly and signed by PIC.

In the Rx, HEB partners can recognize a reportable disclosure incident has occured and HEB Rxists can demonstrate the SOP for reporting disclosures of PHI (protected health info) in accordance with disclosure form process guidelines.
