Mechanism of Action
Estrogens bind to nuclear receptors in estrogen-responsive tissues. Circulating estrogens impact pituitary secretion of gonadotropins, LH & FSH, through negative feedback. Estrogens act to reduce elevated levels in postmenopausal women.
Actions and Uses
Moderate to severe vasomotor symptoms of menopause; vulvar/vaginal atrophy
Prevention of postmenopausal osteoporosis
Adverse Effects
Abdominal pain, back pain, headache, nausea, depression, pruritus, thromboembolism, neoplasm, angioedema
Black Box Warning
Endometrial cancer, breast cancer, CV disorders
Contraindications
Undiagnosed post-menopausal bleeding, ER or PR sensitive tumors, thromboembolic disorders, hepatic impairment
Interactions
erythromycin, clarithromycin, ketoconazole, itraconazole, ritonavir, phenobarbital, carbamazepine, rifampin, St. John’s wort
