oxytocin (Pitocin)
Mechanism of Action
induces labor by increasing frequency and force of uterine contractions
Actions and Uses
- synthetic hormone identical to oxytocin secreted by posterior pituitary gland
- given anterpartum by IV, increases frequency and force of uterine contractions and titrated until normal labor pattern established until normal labor pattern established
- given postpartum to reduce hemorrhage
Adverse Effects
Fetus: dysrhythmias, intracranial hemorrhage, respiratory depression
Maternal: uterine rupture (increased in women who have delivered 5 or > children)
Black Box Warning
Not indicated for elective induction of labor (initiation of labor who has no medical indications for induction).
Contraindications
Cephalopelvic disproportion, fetal distress
Vaginal delivery contraindicated: cervical carcinoma, genital herpes, placenta previa, vasa previa, umbilical cord prolapse
Interactions
vasoconstrictors may cause severe HTN
