mechanism of action
Inhibitory cervical mucus changes including increased viscosity & cell content, with inhibition of sperm transport or migration. Suppresses FSH & LH anovulation Endometrial changes (progestational) which is unfavorable to implantation
Actions and Uses
Contraception
Used in women who are unable to take estrogen
Used most frequently during lactation (estrogen decreases lactation supply)
Adverse Effects
Ectopic pregnancy, ovarian cysts, breast carcinoma, headache, irregular vaginal bleeding
Black Box Warning
Cigarette smoking increases risk of CV adverse effects, risk increases > 35y/o & heavy smoking > 15 cigarettes/day
Contraindications
Breast cancer, undiagnosed irregular bleeding, liver tumors
Interactions
May decrease Progestins: carbamazepine, phenobarbital, phenytoin, rifampin, benzodiazepines, barbiturates
Reduce effect: St. John’s wort
