Class

  • Narcotic Analgesic (schedule II)

Mechanism of Action

  • Opioid receptor agonist
  • Increases tone and decreases smooth muscles of the GI tract

Indications

  • Treat moderate to severe pain
  • Anesthetic for procecural sedation

Contraindications

  • Myasthenia gravis
    • autoimmune neuromuscular disease
  • Children under 2 yo

Adverse reactions

  • Skeletal and thoracic muscular rigidity
  • Bradycardia
  • SZ
  • Diaphoresis

Drug interactions

  • Alcohol and CNS depressants

Dosage and administration

  • Adult:
    • 25-100mcg (0.5-2mcg/kg) IM, IV, IN
  • Ped:
    • 2-12 yo get 1.7-3.3mcg/kg IM, IV

  • Onset
    • IV immediate
    • IM 7-15 min
  • Peak effect
    • IV 3-5 min
    • IM 30 min
  • Duration
    • IV 30-60 min
    • IM 1-2hrs

Special considerations

  • Pregnancy: Cat C
  • May cause severe hypoventilation
  • Narcan as antidote
