-Name, address, telephone # and driver license # of all drivers
-Name, address, telephone # of witnesses
-Name, address, telephone # and policy # of insurance companies
-A copy of the TPD report; at least the report number

When does Form 103 need to be sent to Risk Management after an accident?

Within 24 hours. Unless there is a fatality, brain injury, loss of limb, sight or vision, severe facial scarring, or 3rd degree burns; then contact immediately

-Patient Care Report(s) if applicable
-Memos from all crews members
-Form 103 TFD Vehicle Accident, Lost Property report
-COT Property Damage/Personal Injury Report (if applicable)
-Photographs
-Police Report number at a minimum

What is the paperwork distribution of the Accident Packet?

Fire HR will forward the original COT Property Damage report within 2 days to Risk Management
=Complete copy of the packet into the employee file
=Complete copy of the packet to the Supply Chief
=Complete copy of the packet to the Health and Safety Officer

-Original packet will be kept at Fire Central

What should the front desk personnel do in the case of:

-medical emergency
-security emergency
-police emergency

Push the emergency alert button, which opens up the speakers throughout Fire Central and St 1...

-say "Medical Emergency" Station 1 personnel should respond to the front desk to assist
-say "Security Emergency" all personnel should respond to the front desk to assist
-say "Police Emergency" all personnel should lock down in place until an "all-clear" is given over the speaker

-non injury accidents that are not blocking the roadway
=the driver/owner must stay with the vehicle

can NOT call:
-vehicle accident with injuries and/or blocking the roadway
=responsibility belongs to TPD
-unoccupied vehicles are also the responsibility of TPD
