A

Inhibits sodium/potassium pump, thereby increasing intracellular calcium levels

Positive inotropic effect, increases cardiac output

Decreases chronotropy and dromotropy; slows AV conduction

A

23 VV C

2nd and 3rd degree heart blocks

VFIB VTAC (unless due to CHF)

Use cautiously in patients who have already taken normal therapeutic dose

Digoxin- EXTRAs

A

Precautions: AMI and renal failure

Need to decrease joules by 50% during cardioversion Pregnant or nursing woman Infants and children

May increase risk of dysrhythmias

Use with Beta Blockers may decrease actions of Digoxin

Digoxin has an extremely narrow therapeutic range; therefore, overdose/toxicity is common.

Signs of toxicity include severe sinus bradycardia, 2nd or 3rd degree heart block, VTAC, VFIB, hypokalemia, fatigue, weakness, n/v, loss of appetite, abdominal discomfort, headache and blurred or yellow-colored vision.
