What are the 4 important muscles involved in the movement of the tongue and swallowing?
1) Genioglossus muscle
2) Geniohyoid muscle
3) Mylohyoid muscle
4) Anterior belly of the digastric muscle
Where is the attachment of the Genioglossis muscle?
Superior mental spine
What is the action of the Genioglossus muscle?
Draws tongue forwards and pulls down the central part of the dorsum
Which cranial nerve innervates the Genioglossis muscle?
Hypoglossal nerve
Where is the attachment of the Geniohyoid muscle?
Inferior mental spine
What is the action of the Geniohyoid muscle?
- Elevates and draws hyoid bone forward
- If the hyoid is fixed, depress the mandible
Which cranial nerve innervates the Geniohyoid muscle?
Hypoglossal nerve
- With the nerve fibres derived from C1 spinal nerve
What is the attachment of the Mylohyoid muscle?
Mylohyoid line on medial aspect of mandible, fibres run downwards and medially
What is the action of the Mylohyoid muscle?
- Elevate the floor of the mouth and hyoid
- When the hyoid is fixed, it depresses the mandible
Which nerve innervates the Mylohyoid muscle?
Trigeminal Nerve (V3)
- Mylohyoid branch of inferior alveolar nerve
What does the Mylohyoid have and what is the clinical significant of this?
Posterior Free Ridge
- Infection can spread behind the mylohyoid and come to the superior level
- Pus formation can elevate the mylohyoid, go superiorly to the tongue which can effect breathing
Where is the Posterior fibres of the Mylohyoid muscle attached to?
Body of the Hyoid bone
What is the attachment of the Hyoglossus muscle?
Lateral upper surface of hyoid bone, fibres ascend vertically into side of the tongue
What is the action of the Hyoglossus muscle?
Depresses the tongue
What are the 2 lobes of the Submandibular gland?
1) Superficial Lobe
2) Deep lobe
(Lobes are continuous with each other)
Which artery runs down the Submandibular gland?
Facial artery runs downwards in a deep groove in the lateral surface of the superficial lobe
Describe the duct of the Submandibular gland
- Emerges from anterior surface of deep lobe
- Runs forwards and opens at the sublingual papilla at the base of lingual frenulum
Where does the Sublingual Gland open onto?
The surface of the sublingual fold through number of small ducts (not a single duct)
Describe the function of the Submandibular Ganglion
Parasympathetic innervation of both submandibular & sublingual glands
List the Nerves in the floor of the mouth
1) Lingual Nerve
2) Hypoglossal Nerve
3) Glossopharyngeal Nerve
Where does the Lingual Nerve pass through?
Beneath superior constrictor muscle
What is the Lingual nerve adjacent to?
Medial surface of mandible adjacent to the roots of the third molar tooth
What does the Lingual Nerve supply?
General sensory & (taste) fibres to anterior 2/3’s of the tongue
Where does the Lingual nerve run laterally?
Hyoglossus muscle above the submandibular duct
