A
- Indicated in primary teeth with an advanced proximal or occlusal lesion
- Aim is to completely seal a carious lesion so that the plaque biofilm is altered sufficiently to slow or arrest caries progression
- Technique is only suitable when there is a band of unaffected dentine between the lesion and the pulp

- Procedure
o Ensure child is sitting upright to protect airway and consider use of gauze or ‘sticky sticks’
o Assess whether separators are required – if so place orthodontic separators and then get patient back in 3-5 days later to have them removed and crown fitted
Use floss on both sides and pull tight to seat down through contact points (donut)
Remove with a blunt probe
o Select correct size of PMC – check on tooth but do not fully seat through contacts. Numbers to buccal side
o Load PMC with GI luting cement
o Dry tooth and seat the PMC – apply firm pressure and get child to bite down on the crown or on a cotton wool roll placed on the crown
o Ask child to open and check crown is seated evenly over the tooth and through the contacts – then get child to bite down hard again (bite like a lion)
o Remove excess cement and clear contacts with floss – inform child that it may taste like salt and vinegar crisps
o Reassure patient/parent that high bite will settle and advise on some post-op pain relief
