tamsulosin

alpha 1 antagonist selective for receptors on prostate used to treat BPH by inhibiting smooth muscle contraction

sildenafil, vardenafil

- PDE5 inhibitors, increase cGMP, smooth muscle relaxation in the corpus cavernosum, inc blood flow, penile erection
- tox: headache, flushing, dyspepsia, impaired blue-green color vision. do not take with nitrates

leuprolide

GnRH analog with agonist/antagonist properties
- pulsatile used for infertility
- continuous used for prostate cancer, precocious puberty, uterine fibroids
- tox: antiandrogen, n/v

estrogens (ethinyl estradiol, DES, mestranol)

- bind estrogen receptors
- used for hypogonadism or ovarian failure, menstrual abnormalities, HRT, men with androgen-dependent prostate cancer
- tox: inc risk of endometrial cancer, bleeding in post menopausal women, clear cell carcinoma with DES in utero, inc risk of thrombi
- DO NOT GIVE with ER + breast cancers, or history of DVTs

clomiphene

- antagonist at estrogen receptors in hypothalamus (prevents normal feedback inhibition) and inc release of LH and FSH from pit
- used for infertility due to anovulation (PCOS)
- may cause hot flashes, ovarian enlargement, multiple simultaneous pregnancies, visual disturbances

tamoxifen

- SERM: antagonist on breast tissue, agonist on bone, uterus
- associated with endometrial cancer and DVTs
- used to treat/prevent recurrence of ER+ breast cancer

raloxifene

- SERM: agonist on bone, antagonist on uterus
- increased risk of VTE, decreased resorption of bone
- used to treat osteoporosis

HRT

- used to treat menopause symptoms
- possible increased CV risk

anastrozole/exemestane

aromatase inhibitors used in postmenopausal women with breast cancer
- inhibit synthesis of estrogen in peripheral tissues

progestins

- bone progesterone receptors, decreased growth and increase vascularization of the endometrium
- used in OCPs and to treat endometrial cancer and abnormal uterine bleeding

mifepristone

- competitive inhibitor of progestins at progesterone receptors
- used for termination of pregnancy (given with misoprostol - PGE1)
- tox: heavy bleeding, GI effects, abd pain

terbutaline

B agonist that relaxes the uterus (tocolysis)

danazol

- synthetic androgen that acts as partial agonist at androgen receptors
- used in endometriosis and hereditary angioedema
- tox: weight gain, edema, acne, hirsuitism, masculinization, dec HDL levels and hepatotox

finasteride

- 5a reductase inhibitor
- useful in BPH
- also promotes hair growth (male pattern baldness)

flutamide

- nonsteroidal competitive inhibitor of androgens at the testosterone receptor
- used in prostate carcinoma

ketoconazole

- inhibits steroid synthesis (inhibits 17,20 desmolase)

spironolactone

- inhibits steroid binding, 17a hydroxylase, and 17,20 desmolase
