The colonel may convert excess vacation time credits to sick leave when:
Remains/Returns from ILD
Remains on approved ILD into the next calendar
Returns from approved ILD the last quarter of the current calendar year .
The SP 624 initial injury report must be completed when?
Prior to end of shift and sent via fax to Human Resources occupational health unit or email to the injury email in this policy to general headquarters.
If injured member seek out medical care from their own personal physician:
All bills submitted to the investigating supervisor within 7 days.
All injured members shall complete the following forms within 7 days: (4)
SCAT
S-SP 37, sections, 1-7 statement in support on claim
C-Commonwealth form 1 notice of injury.
A-Authorization of release of medical records
T-To/From (including how the injury occurred in any names of witnesses and statements)

When investigating an injury, investigating officers shall:
Prepare appropriate forms if necessary on injured members behalf (SP 37).
Within 7 days receipt of claim, submit to the Troop/Section/Unit commander: IN SAM
I-Injured members to/from
N-Notice of injury (commonwealth form 1)
S-Statement in support of claim, after completing section 8
A-Any witness statements
M-Medical records and medical bills, if available
Who completes section 9 of the statement and support of claims?
Troop/Section/Unit commander.
Prepare to/from to the president of the board claims which should include a brief account of the facts concerning the injury and conclusions regarding the cause of injury.
All recurrent injuries shall be reported as:
Using the same process as new injuries.
Who shall authorize a member to leave the Commonwealth while on injured leave?
Commanding officer
Who does the commandant of the state police Academy notify once a member has successfully completed required tasks?
Unit commanders
