DVT - what is used as anticoagulation
A
LMW heparin - no monitoring required, eGFR must be >30ml/min - if not use unfractionated
Switch to oral warfarin for 3-5 days
Stop when INR >2.0 for 2 days

How long to use anti-coagulation
A
After 1st DVT - 6 months
After 2nd - lifelong

PE Standard Treatment
A
LMW heparin injections; warfarin for 6 months; IVC filters
Consider DOAC as alternative

Severe PE treatment
A
Thombolysis with tPA; IV unfractionated heparin (monitor APTT)
