.03 LIMITED DUTY ASSIGNMENTS
F. Limited duty assignments are made as available, subject to the following requirements:
1. SAPD Form _____or DWC Form____is required. It is the responsibility of the _____to inform a physician that

officer’s personal physician is unwilling to complete SAPD Form #172 or DWC Form-73, a supervisor may
