Condyloma acuminatum pathogenesis
*caused by the human papilloma virus (HPV)
* is the most common overall STD in women (viral)
*condyloma is associated with HPV types 6 and 11
* HPV subtypes 16 and 18 are associated with cervical and vulvar carcinoma(greater the number ,more associated with cancer )
Condyloma acuminatum Predisposing factors include:
- immunosuppression
- Diabetes
- pregnancy.
Condyloma acuminatum Symptoms
pain, odor, or bleeding
Condyloma acuminatum * Diagnosis
The characteristic appearance of a condyloma is a pedunculated,
soft papule that progresses into a cauliflower-like mass. (key word)
*colposcopy to determine if the lesion extend beyond the visual area
Condyloma acuminatum Management(treatment):
1)Patient-applied topical treatment: imiquimod cream
2) : cryotherapy (liquid nitrogen), or surgical removal
3)pregnant :after 32 week - cryotherapy or electrocoagulation or CO2 laser
4) a biopsy may be indicated to rule out intraepithelial or invasive neoplasia
prevention of HBV
vaccination from :11-26 years (usually )
can be started at 9 years and can be given after 26
indication to biopsy to rule out intraepithelial or invasive neoplasia
*if the cervix is involved
*if the condyloma does not respond to standard treatment
*the patient is postmenopausal (hase greater chance of having underlying vulvar cancer or intraepithelial neoplasia )
*the lesion is pigmented /indurated/fixed/ulcerated
