1-Arterial disease -Assessment:
Look for signs of arterial disease :
* Cool, hairless feet, shiny skin, claudication, Ulcers on heels, outside foot or () toes.

2-Infection:
* Predominant organism → Gram +ve especially Staph.
* In chronic wound → Gram -ve may be found.
* In foot ischeia, gangrene → Anaerobes may be found.
*use proper antibiotics.

3-Measure (grade) the wound
0 → No ulcer
1 → Superficial ulcer
2 → Deep ulcer → without abscess, or osteomyelitis
3 → Deep ulcer + abscess or sepsis of joint or osteomyelitis
4 → Gangrene localized to forefoot or heel
5 → Extensive gangrene

* 4 Corner biopsy for culture & sensitivity
* Dressing: paraffin
