IRIS (Immune Reconstitution Inflammatory Syndrome)
*It occurs in HIV Patient
* Inflammatory disorders with paradoxical worsening of pre-existing latent infections.
* following the initiation of HAART (Highly Active Anti-Retroviral Therapy) in HIV Patients.
* Can occur in response to multiple infections:
* MAI
* Cryptococcus.
* Pneumocystis Carinii.
* Toxoplasma.
* HBV, HCV virus.
Treatment of IRIS
1-In mild to moderate symptoms : Continue HAART
2- In severe cases …. *Discontinue HAART
* Treat underlying infection.
* Consider starting Corticosteroids to decrease inflammation.
Vaccines in adults with HIV
1-Influenza Vaccine (annual كل سنة)
2-Pneumococcal Vaccine
*Pneumococcal conjugate vaccine (PCV13) العميل رقم ١٣: Once for adults with HIV, if not previously received.
*Pneumococcal polysaccharide vaccine (PPSV23)

: Given at least 8 weeks after PCV13, and repeated every 5 years.

4-Hepatitis B Vaccine
5-Hepatitis A Vaccine
6-Tetanus, Diphtheria, and Pertussis Vaccine
*Tdap vaccine once and then Td (tetanus and diphtheria) booster every 10 years.
7-Measles, Mumps, and Rubella (MMR)
8-Varicella Vaccine
9-Human Papillomavirus (HPV) Vaccine
10-COVID-19 Vaccine

If CD4 count is (<200 cells/µL), live vaccines (e.g., MMR, varicella) should be avoided .
