A 48-year-old woman was investigated for fever, right upper quadrant pain. The patient’s symptoms had been continuously increasing for the past 5 years. Ultrasonography and an abdominal computed tomography (CT) were performed. The USG showed a 38 x 29-mm mass. The mass was also found to cause biliary distension in the left lobe. A complete blood count was performed that revealed eosinophilia (2,900/mm3, N: 0-400/mm3, 36% of leukocytes). To establish a definite diagnosis, a biopsy was performed. In the pathological examination, partially granulomatous lesions that consisted entirely of eosinophils and very few mononuclear cells were identified. In the parenchyma, concentric parasite-like structures were observed.

*First-line drug: Triclabendazole
Dose: 10 mg/kg as a single dose (can be repeated once after 12–24 hours if needed).
*Alternative (less effective): Bithionol.

1-Avoid raw aquatic vegetation (e.g., watercress) in endemic areas.

2-Control livestock reservoirs (cattle, sheep).

3-Snail control: Intermediate host (Lymnaea spp.).

4-Public education: Promote safe food practices and hygiene
