Causes
*Amebic:
Entamoeba histolytica (protozoan parasite)
*Pyogenic:
Bacterial infection (often polymicrobial)
*Hydatid:
Echinococcus granulosus (tapeworm parasite)
Common Route
*Amebic:
Feco oral
*Pyogenic:
Biliary tract infection or blood spread spread
*Hydatid:
Ingestion of eggs(dog feces) → portal vein
Onset
*Amebic:
Subacute
*Pyogenic:
Acute
*Hydatid:
Chronic and may discovered accidentally
Symptoms
*Amebic:
Fever, RUQ pain, diarrhea, hepatomegal
*Pyogenic:
Fever, chills, RUQ pain, sepsis
*Hydatid:
asymptomatic; may cause mass effect or rupture
Imaging (US/CT)
*Amebic:
Solitary,hypoechoic, mostly in Rt lobe
*Pyogenic:
Multiple abscesses common; hypodense with rim enhancement Well
*Hydatid:
Well-defined cystic lesion, may show daughter cysts or calcification
Serology
*Amebic:
Positive for E. histolytica antibodies
*Pyogenic:
Nonspecific, cultures often positive
*Hydatid:
Echinococcus serology positive
Aspiration
*Amebic:
Anchovy sauce-like fluid(brown, no pus, sterile)
*Pyogenic:
Pus (culture positive)
*Hydatid:
Clear fluid ± septation; risk of anaphylaxis if cyst ruptures
Treatment
Amebic:
Metronidazole 750 mg 13*10 ± aspiration
*Pyogenic:
Needle aspiration if <5cm,catheter drainage if >5cm
IV antibiotics
*Hydatid:
Albendazole ± surgery or PAIR (puncture-aspiration-injection-
reaspiration)
Complications
*Amebic:
Rupture, secondary bacterial infection
*Pyogenic:
Septicemia, peritonitis.
*Hydatid:
Anaphylaxis, cyst rupture, secondary seeding
Tlc
*Amebic:
No shoft to left
*Pyogenic:
Increase Tlc with shift to left
*Hydatid:
Leucopenia+esinophillia
