What are the causes of hidradenitis suppurative?

abscesses of apocrine sweat glands
anogenital and axillary region
chronic disorder in adults
scarring at site of lesion

double comedones

abscess formation, sinus tracts

What are the scars from this?

hypertrophic scars, keloid scars

What is the pathogenesis of this?

keratin plugging of apocrine duct and hair follicle --> dilation of duct --> inflammation of apocrine gland --> bacterial overgrowth

What bacteria can overgrow in this?

s. aureus, streptococci, e. coli, proteus mirabilis, pseudomonas aeruginosa

What is the treatment drug wise?

PO antibiotics
Tetracycline
Doxycycline
Erythromycin
minocycline
Accutane - early dz

What are some other treatments of hidradenitis suppurative?

intralesional injections with triamcinalone
surgical - marsupializtion, surgical excision, grafting
