NARA destroys records after 2yrs.
BUMED ME/DEN Research Activities transfer records to nearest Fed Record Center (FRC) after 5yrs.
JCAH survey files are destroyed after 5yrs
Healthcare Admin Records
Med/Den OOD logs, event records, journals, smooth logs, destroy after 10yrs.
Rough logs - destroy when 2yrs old.
Med/Den of military members by Non-Federal Facility destroy after 2yrs after final claim.
Quality Assurance Program file - destroy after 5yrs.
DO NOT DESTROY - Punch cards or computers tapes and disc, disposition is not Authorized.
Risk factor screening/ PRT Result - file w/ transfer eval of member and destroy at 5yr.
Fwd Health care treatment - Records to Dept. of Veteran Affairs, Rec Manage. St. MO
Retire inpatient records to NPRC using AHLTA.
At the time of retirement all 3 record will be filed back to back: Inpatient, Fetal monitoring strip, EAR record.
At MTF transfer to NPRC after 1yr: destroy 50yrs old.
Mid shipman dental record - member who enter active duty hand carry record to next duty station.
Mid shipmen that do not enter active duty record are destroyed 5yrs later.
Communicable Disease Case File destroy after 5yrs.
Family Advocacy Case File - transfer to WNRC at 4yrs; destroy at 25yr old.
Pathology & Clinical Lab record - Tissue Exams copies destroy 15yrs old.
Lab findings for inpatient - destroy after 30 days.
Lab findings for out patient - destroy after 90 days.
Lab logs - destroy after 5 yrs.
Prescription form "poly prescription" destroy after 2yrs.
