what are drug therapies for schizophrenia
the most typical treatment for schizophrenia are antipsychotic drugs, they can be taken as tablets or syrup
what are typical antipsychotics
used since 1950s, most common is chlorpromazine,they block dopamine receptors in the brain reducing the action of dopamine reducing the positive symptoms.
what are atypical antipsychotics
drugs that have been used since the 70s, they minimise side effects, an example is clozapine- binds to dopamine receptors but also acts on serotonin-reduces depression, also addresses avolition
what is the A03 for the biological approach
drug therapy is highly effective- thornley et al reviewed studies comparing the effects of chlorpromazine to control conditions in which patients recieved a placebo so their experiences were identical except for the presence of chlorpromazine. data from 13 trials with 1121 pps showed chlorpromazine was associated with better overall functioning and reduced symptom severity. data from 3 trials with 512 pps showed relapse rate was also lower when chlorpromazine was taken.
a limitation are the side effects-typical antipsychptics are associated with dizziness agitation, sleepiness,stiff jaw, weight gain and itchy skin. due to these severe side effects people are more resistant to using drug treatment to cure their schizophrenia
more supporting evidence from meltzer, conducted that clopazine is more effective than typical antipsychotics and other atypical antipsychotics and its effective in 30-50% of treatment resistance cases where typical antipsychotics have failed. this shows the effectiveness of typical antipsychotics
ethica issues-antipsychotics have been used in hospital situations to calm patients and make them easier to work with rather than for the benefits. this is seen as humans right abuse.
how is CBT used to treat schizophrenia
delivery of techniques to identify and manage intrusive or delusional thoughts
patient is encouraged to develop rational interpretations or alternative perceptions e.g. viewing voices as helpful rather than scary
promotes increase in social activity and use of relaxation techniques
what is the a01 for family therapy
takes place with family instead of individual, aim is to reduce anger,frustration and expressed emotion, therapist educates family member about the disorder and what to expect, encourages the family to develop problem solving and communication skills to support the patient.
what is the a03 for the psychological treatment
supporting evidence- jahur et al reviewed the results of 34 studies of CBT for schizophrenia and concluded it has a significant effect on positive and negative symptoms
supporting evidence for family therapy- paroah et al foundd that family therapy reduces hospital readmission and improves quality of life for patients
improves quality of life of patients-cbt allows patients to make sense of and challenge symptoms, family therapy reduces the stress of living with schizophrenia in a family.
has no side effects
ethically questionable-cbt involves challenging a persons paranoia which interferes with freedom of thought, this can modify their politics.
how do you manage schizophrenia with token economies
based on operant conditioning, tokens are distributed anytime patients carry out a desired behaviour targeted for reinforcement, the reward needs to be immediate to prevent delay discounting.
what is the A03 for token economies
supporting evidence-glowacki et al identified 7 high quality studies published between 1999-2013 and examined the effectiveness oftoken economies on people with schizophrenia, all studies shown a reduction of negative symptoms.
ethical issues-priveleges have become more available to patients with mild symptoms and less so for those with more severe symptoms. this means the most severly ill patients suffer from discrimination
