Deformed positon of the hands

PIP is flexed toward palm, DIP is hyperextedned due to rupture of central slip tendon

Treatment for Boutonniere Deformity

Splint PIP in extension but allow MP and DIP flexion
ROM exercises
Surgical repair of tendon and other interventions

Tendons in wrist and thumb (abductor pollicis longus and extensor pollicis brevis) glide through sheath and connect to the base of the thumb

Finkelstein test
Bend thumb across palm of your hand and bend fingers down

*Abnormal thickening of the fascia tissue in the palm that can extend to the fingers
*Firm pits, nodules and chords that pull the fingers into flexion at the MP joints
*Usually not painful

Dupuytren’s Contracture

Causes of Duputren's Contacture

Not known
More common in men over 40 and from northern European descent

Test for this is Palpitation

*Collagenase injected into the tissue
*Needle aponeurotomy to cut the tissue
Skin graft may be needed
Splinting and hand therapy to maximize and maintain hand position and function

Treatment for Duputren's Contracture

Damage to the ulnar collateral ligament (stretched or torn) of the MP joint (UCL prevents the thumb from being pulled too far away from the hand)
