7

Hutchinson teeth?

Weird looking teeth from congenital syphilis.

Treat early.

8

How do you recognize congenital syphilis in clinic?

Snuffles: bloody drips from nose.

Treat it and may prevent outcomes like saber shins, hutch teeth, mulberry molars, saddle nose...

9

What are VDRL/RPR?

Recognize serologic find antibodies in serum.

Can do false positive and confirm with FTA-ABS which is specific

10

Tx of primary syphilis?

Penicillin G, if allergic, use doxy

11

VDRL false positive?

VDRL

Virus
Drugs
Rheum fever
Lupus

Confirmwith FTA-ABS

What is a ghon complex?

Hilar lymph and ghon focus on x ray. Primary Tb

Primary tb vs secondary in the lung?

Primary is ghon complex in mid lung

Secondary is cavitation with cavitary caseous necrosis in upper lobes and more symptoms

27

What bacteria often goes to back?

mycobacterium Tb in pott diseaes in extrapulmonary secondary Tb

Also can ht CNS, lymph, renal, GI, adrenal

28

Mechanism for drug for latent tb? What do you add to reduce toxicity?

INH is blocking synth of my colic acid and needs to be actriavatvie by catalase

pyridoxine helps with neuro and hepatotoxicity

Also causes lupus

29

Primary or secondary Tb tx?

RIPE

Rifampin
Isoniazid
Pyrazinamide (acidifies lysosomes of macrophage)
Ethambutol (less carb poly of mycobacterium cell wall and optic neuropathy)

30

Two kinds of leprosy? Tx for each?

Lepromatous is worse and communicable. Humoral response Th2

Tx: Dapsone, rifampin, clofazimine for 2-5 years

Tuberculoid: Tb some hairless skin plaques and cell0mediated immunity with Th1

Tx: dapsone and rifampin

31

Dapsone toxicity? When is it used?

Hemolysis in G6PD

PCP pnuemonia (in addition to TMP SMX)
Lepromatous

32

When is rifampin used?

Tb
Leprosy
Meningitis prophylaxis: menigiococcal, H influenzae type B

33

Toxicity of RIPE regiment?

basically look out for hepatotoxicities

34

Gardnerella symptoms and tx?

Vaginosis: gray discharge and fishy smell Clue cells!

Metronidazole

35

Ricketsia triad and tx?

Headache fever and rash

Tx: Doxy

36

Ricketssia Rickets starts on wrRists and spreads in and includes soles

R typhi stratrts on Trunk and moves out sparing soles (also prowazeki)

Epidemic is outbreaks from louse (lice) and prowazekii

Both start in central and spread but spare soles/palm

38

What is Q fever? what causes it?

Q fever is from Coxiella burnetii and similar to rickets

No rash or vector and is from cow placenta and has spores inhaled as aerosols and causes fever and pneumonia

Tx with doxy

What is weil felix reaction?

for rickets antibodies. Doesn't work for Q fever (coxiella burnetii)
