What is varicocele?
Dilation/congestion of pampiniform plexus draining the scrotum
Varicocele is characterized by the enlargement of the veins within the scrotum.
Does varicocele transilluminate?
No, it does not transilluminate
This is in contrast to hydroceles and spermatoceles which do transilluminate.
How can varicocele be described visually?
As a buzzy ‘bag of worms’
This description refers to the appearance of the dilated veins.
What sensation might a patient with varicocele report?
A heavy, dragging scrotum lower on the left
This sensation is often reported due to the weight of the dilated veins.
On which side does varicocele almost always occur?
Left side
This is primarily due to the anatomy of venous drainage.
What is the pathway of the left testicular vein?
Left testicular vein to left renal vein to IVC
This pathway contributes to the higher incidence of varicocele on the left side.
What is the pathway of the right testicular vein?
Right testicular vein to IVC (right to IVC)
This more direct pathway reduces the risk of varicocele on the right side.
Why does the left testicular vein have a proclivity for hydrostatic pressure backup?
It drains at a 90-degree angle into the left renal vein
This anatomical feature increases the likelihood of varicocele on the left.
What kidney related scenario can lead to varicocele?
Left renal vein thrombosis in setting of nephrotic syndrome
hypercoagulable state due to loss of antithrombin III in the urine.
Note about varicocele sidedness
Can be bilateral
