Amphotericin B, Nystatin

Binds ergosterol creating holes in membrane

Kills yeast and mold
Resistance is rare
Binds to cholesteral and causes NEPHROTOXICITY
*can cross blood brain barrier

Fluconazole
Itraconazole
Ketoconazole

binds fungal p450 enzyme and prevents production of ergosterol.
Resistance- alter p450 enzyme
-may cause hepatotoxicity and nephrotoxicity
-AVOID WHEN PREGO
**Most widely used

Terbinafine

blocks production of ergosterol by inhibits squalene epoxidase.
-kills dermatophytes
-resistance is rare
-drug interactions with Cyp 2D6

Fluctosine (5-FC)

selectively taken up by fungi and converted to 5 fluorouracil in fungi. Interferes with DNA/RNA synthesis
-YEAST ONLY
-can lead to bone marrow suppresion
-TREAT W/ AMPHOTERICIN B

Griseofulvin

-binds microtubules and inhibits sliding
-fungistatic for dermatophytes
-Resistance= change in B-tubulin, if patient isn't complient may develop
-AVOID WHEN PREGO
-lipids increase oral absorption

Caspofungin

cell wall inhibitor, blocks synthesis of glucan
-Fungicidal for candida
-Treats Candida albicans
-Resistance = change in glucan synthase gene
-may get fever or rash as site of injection
-Have to give through IV
