Severity of Burns

A

1st degree - blanchable redness (superficial)
2nd degree - blisters, wheeping, edema (partial thickness)
3rd degree - subcut, edema, eschar, pain reduced
4th degree - Muscle and bone involved, eschar, severe edema, no pain (full thickness)

A
  • LR for fluid (Lots of it! 4ml x kg x %burn over 24hr)
  • IV analgesic
  • tetanus shot
  • foley for strict I/O
  • normothermia

Burn Complications (9)

A
  • Hyperkalemia
  • Dehydration
  • Compartment syndrome (tight skin and/or third spacing increases pressure on vessels)
  • Hypovolemic shock
  • Hyperglycaemia (increased cortisol)
  • Inhalation injury
  • carbon monoxide poisoning (false SPO2 reading) blast O2
  • Burn sepsis
  • ARDS
