Aminoglycoside family

A

Abx ending in “mycin”
- excluding “THROmycins” such as erythromycin
- A MEAN OLD MYCIN = for severe, deadly infections
COMPLICATIONS
- Ears, such as hearing loss, tinnitus, vertigo. (think mycin=mice=big ear)
- Nephro complications (monitor Cr), (ear and kidney same shape)
- Cranial nerve 8, because an 8 fits in ear shape nicely

Calcium Channel Blockers

A

End in dipine (also vermapril, cardizem)
- They relax and slow down the heart and lower BP (negative Ino, Chrono, Dromotropic)
- A, AA, AAA (antihypertensive, anti anginal, anti atrial arrythmia
- Parameters: hold if BP<100

A

Antimania drug used for Bipolar, specifically manic episodes.
Therapeutic level 0.6-1.2
Toxic level > 2
- pt must be hydrated, do not take with NSAIDs
SE (3Ps)
- Polyuria
- Pooping
- Paresthesia
TOXIC SE
- Tremors
- Metallic Taste
- Severe Diarrhea

Digoxin/Lanoxin

A

Used to treat HF and a-fib. Decreases HR
Therapeutic level 0.5-2
Toxic level >2
- Do not give if HR <90
- Moniotr K levels (hypokalemia can cause toxicity)

- Major tranquilizers
- In small doses can be used as antiemetics

- Anticholinergic (dry mouth, urinary retention)
- Blurred vision
- Constipation
- Drowsiness
- EPS (tremors, like parkinsons)
- Fotosensitivity
- aGranulocytosis (low WBC, immunosupressed)

A

Long acting form of a drug
- IM for non-complicance
- Often court ordered

Tricyclic Antidepressants

A

Mood elevators, grnadfathered into the NSSRI group (Non-selective serotonin reuptake inhibitors)
E.g: Elavil, Trofranil, Aventyl, Desyrel
- Onset 2-4 weeks
SE (ABCDE)
- Anticholinergic (dry mouth)
- Blurred vision
- Constipation
- Drowsiness
- Euphoria

Benzodiazepines

A

Anti-anxiety, minor tranquilzers. Always have ZEP in the name, and sometimes end in LAM or PAM
Side uses:
- seizure meds
- pre-op induction of anesthesia
- muscle relaxant
- EtOH withdrawal
- Ventilation (calms pt on vent)
SE:
- Anticholingeric
- Blurred vision
- Constipation
- Drowsiness

Monamine Oxidase (MAO) inhibitors
Antidepressants (brand names start with MAR, NAR, PAR. e.g MARplan, NARdil, PARnate) SE: - Anticholingeric - Blurred vision - Constipation - Drowsiness TEACH: avoid foods with tyramine - salad BAR: banans, avocado, raisin (all dried fruit) - Yeast - Organ or preserved Meats - Dairy (cottage cheese and mozarella ok) - EtOH

Prozac/ Fluoxetine
SSRI, mood elevator - cause insomina so give before noon - when changing or starting doses, it can cause suicidal tendencies for youth and young adults SE - Anticholinergic - Blurred vision - Constipation - Drowsiness - Euphoria

Haldol/haloperidol
1st generation, tranquilizer. - May develop NMS (neuropletic malignant syndrome, AKA high fever, and) SE - Anticholingeric - Blurred vision - Constipation - Drowsiness - EPS (tremors like parkinsons) - Fotosensitivity - aGranulocytosis (low WBC)

Clozaril/Clozapine
Atypical antipsychotic. - DONT confuse with clonazepam SE: - aGranulocytosis!! Worse than cancer meds, will trash pt bone marrow

What patients CANNOT get geodon (ziprasidone)
Pt with cardiac conditions, it causes a prolonged QT interval

Zoloft/Setraline
Antidepressant - Can cause insomnia - Interacts with St Johns Wort (serotonin syndrome) and Warfarin (bleeding) SE of serotonin syndrome (SAD Head) - Sweating - Apprehensive (impending doom) - Dizzyness - Headache
