What is the pathophysiology of FATE?
Thrombus originates in left heart and dislodges to systemic arterial system
What are 5 physical exam findings in a patient with FATE?
Pulselessness
Palor
Pain
Paresis
Polar/Poikilothermia (cold)
What diagnostics may be helpful in diagnosing FATE?
High lactate, low glucose on affected limb
Lack of doppler BP
Increased CK (and AST/ALT)
Cardiac POCUS, echocardiogram
What treatment strategies are used in the setting of FATE?
Analgesia is priority (fentanyl)
Anti-thrombotics - heparin (enoxaparin), oral factor Xa inhibitor (apixaban or rivaroxaban)
Anti-platelets - clopidogrel
Patient will forever be on clopidogrel and anti-thrombotic
What is the prognosis for patients with FATE?
1/3 will be euthanized
1/3 will succumb to disease
1/3 will go home
What are some possible complications of treating FATE?
Hyperkalemia (reperfusion injury) - monitor electrolytes!
Limb necrosis
What are the recommended rechecks for FATE?
3-10 days after discharge
2 weeks after discharge
q1-3 months
