For USMLE purposes, DLCO is decreased in every pathologic lung condition, but increased in

asthma
This is thought to be due to a combination of transient increase in blood flow through the pulmonary capillaries + additional hyperinflation during acute episodes.
In other words, if you get a big paragraph Q where you’re not sure what’s going on, but they
say DLCO is reduced, you can say, “Cool, not asthma.” Or if they say DLCO is high, you can just select asthma without even reading the question basically.
